Biopsym : a learning environment for transrectal ultrasound guided prostate biopsies
This paper describes a learning environment for image-guided prostate
biopsies in cancer diagnosis; it is based on an ultrasound probe simulator
virtually exploring real datasets obtained from patients. The aim is to make
the training of young physicians easier and faster with a tool that combines
lectures, biopsy simulations and recommended exercises to master this medical
gesture. It will particularly help acquiring the three-dimensional
representation of the prostate needed for practicing biopsy sequences. The
simulator uses a haptic feedback to compute the position of the virtual probe
from three-dimensional (3D) ultrasound recorded data. This paper presents the
current version of this learning environment

Étude didactique de la dialectique du travail et de la formation au bloc opératoire
Cet article présente une étude didactique de la formation professionnelle des internes au bloc opératoire. Une spécificité importante des interactions entre l’apprenti et le maître dans ce contexte est qu’elles se déroulent en situation de travail : les enjeux professionnels et les enjeux de formation doivent coexister. Nous centrons cette étude sur la description de la nature des interactions et de la nature des contenus sur lesquelles elles portent. Ces deux entrées nous permettent d’identifier un contrat didactique particulier, mais néanmoins proche de celui étudié en didactique des disciplines, et qui semble permettre la coexistence, dans un équilibre qui varie, de ces deux enjeux à première vue incompatibles.This paper presents a didactical study of apprenticeship in operating theatre. One specific aspect of the interactions between the apprentice and the master in this context is that they occur during work : professional and training issues have to coexist. Our study describes the kind of interactions that occur and the kind of content they deal with. These two entries allow us to identify sort of a didactical contract, like the one which is studied in didactics of disciplines. This contract seems to allow the coexistence, in a variable balance, of the two previous mentioned issues, which are at first glance incompatible
BiopSym: a simulator for enhanced learning of ultrasound-guided prostate biopsy
This paper describes a simulator of ultrasound-guided prostate biopsies for
cancer diagnosis. When performing biopsy series, the clinician has to move the
ultrasound probe and to mentally integrate the real-time bi-dimensional images
into a three-dimensional (3D) representation of the anatomical environment.
Such a 3D representation is necessary to sample regularly the prostate in order
to maximize the probability of detecting a cancer if any. To make the training
of young physicians easier and faster we developed a simulator that combines
images computed from three-dimensional ultrasound recorded data to haptic
feedback. The paper presents the first version of this simulator
Initial validation of a virtual-reality learning environment for prostate biopsies: realism matters!
: Introduction-objectives: A virtual-reality learning environment dedicated
to prostate biopsies was designed to overcome the limitations of current
classical teaching methods. The aim of this study was to validate reliability,
face, content and construct of the simulator. Materials and methods: The
simulator is composed of a) a laptop computer, b) a haptic device with a stylus
that mimics the ultrasound probe, c) a clinical case database including three
dimensional (3D) ultrasound volumes and patient data and d) a learning
environment with a set of progressive exercises including a randomized 12-core
biopsy procedure. Both visual (3D biopsy mapping) and numerical (score)
feedback are given to the user. The simulator evaluation was conducted in an
academic urology department on 7 experts and 14 novices who each performed a
virtual biopsy procedure and completed a face and content validity
questionnaire. Results: The overall realism of the biopsy procedure was rated
at a median of 9/10 by non-experts (7.1-9.8). Experts rated the usefulness of
the simulator for the initial training of urologists at 8.2/10 (7.9-8.3), but
reported the range of motion and force feedback as significantly less realistic
than novices (p=0.01 and 0.03 respectively). Pearson's r correlation
coefficient between correctly placed biopsies on the right and left side of the
prostate for each user was 0.79 (p<0.001). The 7 experts had a median score of
64% (59-73), and the 14 novices a median score of 52% (43-67), without reaching
statistical significance (p=0,19). Conclusion: The newly designed virtual
reality learning environment proved its versatility and its reliability, face
and content were validated. Demonstrating the construct validity will require
improvements to the realism and scoring system used
Une caractérisation des indicateurs élaborés dans l’activité. Perspectives sémiotiques sur la formation professionnelle
Nous nous intéressons ici à la notion d’indicateur dans le cadre de l’analyse de l’activité pour la formation. Cette notion permet, en échappant au piège dualiste, de concevoir un individu qui n’a pas un réel objectif sous les yeux, mais plutôt des éléments concrets potentiellement signifiants (signes ou indicateurs potentiels). Cette lecture sémiotique n’est pas exclusive, et doit être articulée avec les approches théoriques qui occupent le champ de la didactique professionnelle (l’articulation au constructivisme est en particulier défendue). Nous présenterons trois catégories d’indicateurs, et les questions inhérentes à leur validité. Nous développerons une « méthode des indicateurs » à travers la caractérisation des indicateurs en cinq dimensions : la relativité, la subjectivité, la sûreté, la responsabilité et la convertibilité. Chacune de ces dimensions relie les modalités du processus interprétatif aux aspects de l’activité qu’ils risquent d’influencer (par exemple, si la sûreté de la chaine indicielle doit toujours être suspectée, elle ne l’est, dans les faits, que pour certaines valeurs de l’indice). Nous mettons en évidence ces rapports par des exemples dans le domaine de la maïeutique (par exemple, les étudiantes sages-femmes sont rassurées par la technicité d’une chaîne indicielle longue – imagerie ou tracé – alors que les sages-femmes expérimentées cherchent à vérifier par accès direct – palpation ou toucher vaginal – la sûreté d’une imagerie ou d’un tracé).Our subject of interest is the notion of indicator in the domain of analyzing the activity with a training point of view. While escaping the dualist trap, this concept allows conceiving an individual who doesn’t lay eyes on an objective reality, but observes concrete elements which are potentially significant (potential indicators or signs). This semiotic rendition is not exclusive, and must be articulated with the theoretical approaches of the field of vocational didactics (we particularly underline the articulation with constructivism). We present three categories of indicators and the inherent question of their validity. Finally, we develop a “method of indicators” through their characterization in five dimensions: relativity, subjectivity, safety, responsibility and convertibility. Each of these dimensions connects the modalities of the interpretative process to aspects of the activity that they are likely to influence (for example, if the safety of the index chain should always be suspected, in reality, it is done only for specific values of the index). We highlight these connections by examples in the field of maieutic (for example, midwifery students are reassured by the technical nature of a long index chain while experienced midwives seek to verify by a direct access the safety of an imagery or tracing)
TELEOS : de l'analyse de l'activité professionnelle à la formalisation des connaissances pour un environnement d'apprentissage

Cette modélisation permet la prise en compte dans l'environnement informatique de connaissances empiriques pour le diagnostic des connaissances de l'utilisateur en fonction des actions qu'il effectue à l'interface pendant la résolution d'un problème (pose de vis dans le bassin), et la prise de décision didactique qui suit : quelle rétroaction fournir pour affiner le diagnostic, et/ou permettre l'apprentissage souhaité. Nous présentons ainsi l'analyse, le formalisme et la représentation des connaissances pour la production d'un EIAH (Environnement informatique pour l'apprentissage humain) en milieu professionnel
Apprentissage de la chirurgie orthopédique assisté par ordinateur : Le cas du Système Tutoriel Intelligent TELEOS
The purpose of the research presented in this paper is to design a computer-assisted human learning environment. The particular case under study is the intelligent tutoring system TELEOS (Technology Enhanced Learning Environment for Orthopaedic Surgery), which supports the learning of percutaneous orthopedic surgery, and specifically, vertebroplasty and iliosacral screw fixation. The paper summarizes all of the research work underpinning the system’s development, current functionalities, and upcoming modifications. The benefits of using this intelligent tutoring system as a support for conventional training and leaning methods are also discussed.L’objet de la recherche prĂ©sentĂ©e dans cet article est la conception d’Environnement Informatique pour l’Apprentissage Humain. Conceptions de l’angle chez des élèves de seconde
This article reports a study of the conceptions of angle possessed by 15-year-olds who had not studied the notion of directed angle. The theoretical framework-in particular, the notion of conception-was that described in Balacheff (1995 a, b). We initially studied some aspects of the notion of angle as slope. This interpretation has been much neglected in the educational research literature and in instruction despite its great historical importance (it was Euclid’s definition) and its practicality (e.g., it is used by topographers). We continued by examining current textbooks so as to define the spheres of practices that students associate with the notion of angle. Finally, we present the design of an experimental device that we used to test our hypothesis about the conceptions of angle available to students in a problem-solving situation with Cabri Geometry II. It was a communication situation that involved changing the size of a pentagon without recourse to measuring lengths. The results, illustrated by a case study, show that a conception of angle as slope is accessible to pupils, although they first mobilize angle conceptions that are closely linked to the direct characteristics of the figure (edges and sides).Le sujet de cet article est l’étude de conceptions de l’angle chez des élèves de seconde n’ayant pas encore abordé la notion d’angle orienté. Le cadre théorique, en particulier pour la notion de conception, est celui décrit par Balacheff (1995a et b). Nous étudions dans un premier temps quelques aspects de l’angle en tant qu’inclinaison, sujet peu traité dans la littérature et dans l’enseignement malgré son importance tant historique (c’est l’angle que définit Euclide) que pratique (il est utilisé par les topographes, par exemple). Nous poursuivons par une étude de manuels scolaires qui nous permet de définir les sphères de pratique associées aux angles chez les élèves. Enfin, nous présentons la mise en place d’un dispositif expérimental destiné à valider nos hypothèses sur les conceptions de l’angle disponibles chez les élèves dans une situation de résolution de problème sur Cabri-géomètre ii : il s’agit d’une situation de communication portant sur la reproduction, dans une taille différente, d’un pentagone, sans recourir à la mesure des longueurs. Les résultats, illustrés par une étude de cas, montrent qu’une conception de l’angle en tant qu’inclinaison est accessible aux élèves, bien qu’ils mobilisent d’emblée des conceptions davantage liées aux caractéristiques directes de la figure
